Modify Evaluation Forms
You must have the default Administrator or Supervisor role or a custom role that includes the following permissions to modify evaluation forms:
- Quality > Evaluations > Full Control or
- Quality > Evaluations > Modify
- Quality > Evaluation Forms > Full Control or
- Quality > Evaluation Forms > Modify
Unlike modifying Form Options, an evaluation form can be modified even when it is saved. However, a few of the questions and scoring features are not modifiable if the Evaluation Form is In Use. This chart defines what is modifiable during any state of an evaluation:

- From the Evaluation > Forms tab, select an evaluation form to modify.
- If the form is an Active form that is not In Use, click the Activate/Deactivate icon in the ribbon bar to deactivate the form. This ensures that the form cannot be used by an evaluator while it is being modified. If it is, then any changes made will not be saved.
- Click the Modify Form icon in the ribbon bar. The Modify Evaluation window appears.
- Modify the form as desired. The entire form is open for modification:
- Evaluation Info
- Summary
- Question Groups
- Questions
- When finished modifying the Evaluation Form, click on the Save Form and Close Form icons in the ribbon bar.
- From the Evaluation > Formstab, select an evaluation form to modify.
- Click the Modify Form icon in the ribbon bar. The Modify Evaluation Form window appears.
- Only the following settings can be modified in an In Use form:
- Evaluation Form Settings:
- Modify the form’s state from active to inactive,
- Set whether Only the Owner can Modify
- Activate and deactivate the Auto-Fail features
- Set a locking time for the form
- Header Settings: Select which Header fields to include in the form including newly created custom Header Categories and Fields.
- Scoring Header: Select the whether the point values are hidden or displayed. The weighting system will not be available to be modified.
- Questions: After activating Auto-Fail settings, chose which questions may constitute an Auto-Fail; and update any Evaluator Help text.
- All other question and question group options will be grayed out and unavailable to be modified.
- Evaluation Form Settings:
- When finished modifying the Evaluation Form, click on the Save Evaluation and Close Evaluation icons in the ribbon bar.
If any other modification needs to be made to the questions, question groups or scoring, a new form will need to be created. Use Copy Form to replicate the exact same form and make the necessary modifications.
Question Groups can be created in or out-of-order. Users can rearrange the question groups to make the evaluation form more effective by using the drag-and-drop feature. This means the user can click on one question group button and, while holding the mouse button down, drag-and-drop the question group to another place within the evaluation form.
