Create a New Header Field

You must have the default Administrator role or a custom role that includes the following permissions to create new header fields:

  • Quality > Evaluations > Full Control
  • Quality > Evaluation Forms > Full Control
  1. A new Header Field can be created from any of these two screens:
    1. Evaluation Form Management window located at Evaluation » Forms » Header Fields
    2. Create Evaluation Form window located at Evaluation » Forms » New Form icon » Header Fields icon (as well as Copy Form and Modify Form)
  2. A pop-up window titled Manage Header Fields appears with a mini-ribbon bar. Click the New icon to configure a field for the header.
  3. Another pop-up window called New Header Field appears. Enter or copy/paste a name in the Name textbox that is easily recognizable. The text box will accept up to 50 characters.
  4. From the Data Validation drop down box, select one of these appropriate types that best describes the data that will be input into the Header Field.
    1. Text – Evaluators can enter a combination of up to 100 letters, numbers, and special characters in the Header Field.
    2. Whole Number – Evaluators can enter only positive and negative numbers into a Header Field using the whole number validation. Numbers accepted in the field range from: -9223372036854775808 to 9223372036854775807. No decimal numbers will be accepted.
    3. Date – Evaluators can select a date from a pop-up calendar or input a date in the MM/DD/YYYY format.
    4. Time – Evaluators can select a time by using the up and down arrow in the time selector field or input a time in HH:MM:SS.
  5. Click OK to save.
    1. If the category name already exists in the database, then the “Header Field Name already exists” error will pop-up. Choose a different name for the category.
    2. If the category name was left blank, then the “Name is Required” error will pop-up.
    3. If the data validation was not selected, then the “Data Validation is required” error will pop-up. Select a Data Validation choice.
  6. The new header field will be listed in the Manage Header Fields window.
