View and Copy Forms

You must have the default Administrator or Supervisor role or a custom role that includes the following permissions to view evaluation forms:

  • Quality > Evaluations > Full Control or
  • Quality > Evaluations > View
  • Quality > Evaluation Forms > Full Control or
  • Quality > Evaluation Forms > View

View Forms

To view the content of a form without the risk of accidentally changing it:

  1. From the Evaluation > Forms > Evaluation Form Management window, select an evaluation form from the list of forms for viewing.
  2. Click the View Form icon in the ribbon bar or double click on the form's record. The View Evaluation Form window will appear.
  3. All Form Settings, Question Groups, Questions and Settings can be viewed but are grayed out and unmodifiable.
  4. When finished viewing the Evaluation Form, click on the Close Form icon in the ribbon bar.

Copy Forms

An existing evaluation form can be copied in order to modify an In Use Evaluation Form or to shorten the form creation effort by creating a new form with similar scoring and questions. To copy an existing Evaluation Form:

  1. From the Evaluation > Forms tab, click on the record of the form desired to be copied.
  2. Click the Copy Form icon in the ribbon bar. The Copy Evaluation Form window will appear.
  3. The form will be copied exactly except the form name will have “Copy of” in front of the original form name. The copied form includes the following of the original form:
    • All Evaluation Form Settings (except the Form Name)
    • All Header Settings
    • All Scoring Header Settings
    • All Question Groups
    • All Questions including Form Options, Points, Question Rules and Tips
  4. Rename the Form Name of this copy. Add a version number, if needed.
  5. Make any modifications necessary to the copied form. Changes will not affect the original form.
  6. Click Save Form in the ribbon bar when all changes have been made.
  7. Click Close Form when finished with the form.
